Intake Form

Please fill out our intake and policy forms before you purchase and register for Rover-Achiever.

Max age limit 4 months unless received prior approval from instructor

I will keep my puppy home if s/he is:

  • showing any signs of illness, including but not limited to: cough, vomiting, diarrhea, and/or fatigue. 

  • currently undergoing treatment or on medication for illnesses or internal parasites

My puppy is up-to-date with all age-mandated vaccinations, including a minimum of:

  • 1 round of Distemper/Parvovirus vaccinations (e.g. DHLPP, DHPP, DA2PP, DAPP)

  • at least 7 days prior to their first class

I acknowledge that class size is limited so that all attendees can receive appropriate time and attention.

If I need to cancel, I will notify Treats and Triumphs Dog Training LLC via email at least 48 hours in advance so another puppy has the chance to attend.

I understand that there are no refunds for late cancellations and no-shows. 

I understand that Treats and Triumphs Dog Training LLC is a force-free, fear-free company and does not allow choke chains, prong collars, or electronic collars in class. My puppy will wear a flat buckle, quick-snap closure, or martingale collar, and/or a harness or head halter.

I recognize that retractable leashes are not permitted in class for safety reasons. I will bring a flat, 6-foot leash.

I understand that a photographer may be present during class, and that Treats and Triumphs Dog Training LLC will share the resulting photos with attendees for personal use and may use select images to promote their services and raise interest in the foster puppies who attended.

I give Treats and Triumphs Dog Training LLC permission to publish in print, electronic, or video format the likeness or image of me and/or my dog.  I release all claims against Treats and Triumphs Dog Training LLC with respect to privacy rights, publicity rights, copyright ownership and publication, including any claim for compensation related to use of the materials.

I understand that Treats and Triumphs Dog Training LLC will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. I have been told by Treats and Triumphs Dog Training LLC and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others, and consequently I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify, release, and hold harmless Treats and Triumphs Dog Training LLC of any and all claims, whether made by myself or any third party of injury, expense, costs, or damages caused by my dog. 

I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service. 

I hereby agree to abide by the rules and policies of Treats and Triumphs Dog Training LLC training classes as set forth in this contract. I understand that attendance of dog training classes is not without risk to myself, members of my family, guests who may attend, or to my dog.  I hereby agree to indemnify, release, and hold harmless Treats and Triumphs Dog Training LLC its officers, directors, instructors, agents, employees and/or representatives of any and all claims made by myself or any member of my family, or accompanying guests of mine, or any other third party, of injury, expense, costs or damages to myself, my dog or any handler sponsored by me both in class and out of class. In addition, I agree that I will defend and indemnify Treats and Triumphs Dog Training LLC for any injury, expense, costs or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog.
